Per the Ohio State Board of Pharmacy, these methods require additional checks and will significantly slow processing of your prescription. We therefore highly recommend having your doctor fax your prescription to 1-866-821-3784.1. PHONE - Have your doctor's office call us at 1-866-885-0508.
2. MAIL - You may mail the prescription you received from your doctor directly to us at: HealthWarehouse ATTN: PHARMACY, 100 Commerce Blvd. Cincinnati, OH 45140.
3. TRANSFER - Email or call us with your pharmacy phone number and prescription number to pharmacy@healthwarehouse.com. We will contact your pharmacy to intiate transfer of your prescription. In some cases, pharmacies may refuse to transfer your prescription in which case we must contact your doctor's office directly.
